Zollinger-Ellison Syndrome

What is Zollinger-Ellison Syndrome?
Zollinger-Ellison Syndrome (ZES) is a rare disorder characterized by the formation of one or more tumors called gastrinomas, usually in the pancreas or duodenum. These tumors secrete excessive amounts of the hormone gastrin, leading to overproduction of stomach acid. This causes severe and recurrent peptic ulcers and other digestive problems.

What Causes Zollinger-Ellison Syndrome?

  • Gastrinomas – Tumors that produce excess gastrin hormone, which stimulates the stomach to produce too much acid.
  • Genetic Factors – Some cases are associated with multiple endocrine neoplasia type 1 (MEN1), an inherited disorder causing tumors in endocrine glands.

What Are the Symptoms of Zollinger-Ellison Syndrome?

  • Abdominal Pain – Often severe and related to peptic ulcers.
  • Diarrhea – Due to excessive stomach acid affecting digestion.
  • Nausea and Vomiting
  • Gastroesophageal Reflux Disease (GERD) – Heartburn and acid reflux.
  • Bleeding Ulcers – Leading to black or bloody stools or vomiting blood.
  • Weight Loss – Due to poor nutrient absorption and chronic illness.

How is Zollinger-Ellison Syndrome Diagnosed?

  • Blood Tests – Elevated gastrin levels in the blood, especially after fasting.
  • Secretin Stimulation Test – Used to confirm gastrinoma by measuring gastrin response.
  • Imaging Tests – CT scan, MRI, endoscopic ultrasound, or somatostatin receptor scintigraphy to locate tumors.
  • Endoscopy – To check for ulcers in the stomach and duodenum.

How is Zollinger-Ellison Syndrome Treated?

  • Proton Pump Inhibitors (PPIs) – Medications to reduce stomach acid production and heal ulcers.
  • Surgical Removal of Tumors – If possible, to cure or control the condition.
  • Management of Complications – Such as bleeding ulcers or infections.
  • Treatment of MEN1 – If the syndrome is part of this genetic disorder, management includes monitoring and treating other endocrine tumors.

Prognosis of Zollinger-Ellison Syndrome
With proper treatment, symptoms can often be controlled. However, some gastrinomas may be malignant and can spread, requiring ongoing monitoring and treatment.
