How Is Prostate Cancer Detected?

Prostate cancer is one of the most common cancers in men, especially those over 50. Early stages often have no symptoms, so regular screening is important for early detection and successful treatment.

Common Methods to Detect Prostate Cancer

1. Prostate-Specific Antigen (PSA) Blood Test

  • This test measures the level of PSA, a protein produced by the prostate.
  • Elevated PSA levels may indicate prostate cancer, but can also be caused by non-cancerous conditions like prostatitis or an enlarged prostate.
  • It is often the first step in screening.

2. Digital Rectal Exam (DRE)

  • A doctor gently inserts a gloved, lubricated finger into the rectum to feel the size, shape, and texture of the prostate.
  • Hard or irregular areas may raise concern and lead to further testing.

3. Imaging Tests

  • Transrectal Ultrasound (TRUS): Uses sound waves to create images of the prostate. Often used during biopsy procedures.
  • MRI Scan: May be used to better visualize suspicious areas before a biopsy.

4. Prostate Biopsy

  • If PSA or DRE results are abnormal, a biopsy is done to confirm if cancer cells are present.
  • A needle is used to take small samples of prostate tissue, typically guided by ultrasound.

5. Newer Biomarker Tests

  • Some advanced tests help differentiate between aggressive and slow-growing cancers, improving diagnosis and reducing unnecessary biopsies.

When to Get Tested

  • Men aged 50 and above should discuss screening with their doctor.
  • Those at higher risk (e.g., African descent, family history of prostate cancer) may consider testing starting at age 40–45.

Symptoms That May Appear in Later Stages

Although early prostate cancer often has no symptoms, later signs can include:

  • Difficulty urinating or weak urine flow
  • Blood in urine or semen
  • Pain in the hips, back, or pelvis
  • Erectile dysfunction
  • Unexplained weight loss
